If you or your spouse receives a citation for a non-criminal1 moving or point violation in either your commercial or personal vehicle, your Provider Law Firm will represent you in the court of original jurisdiction. IRS Audit Legal Services (Including Schedule C) Don't face an IRS audit alone! If you or your spouse receives written notice of an IRS audit of your Schedule C tax return, you can receive up to 50 hours of professional services from your Provider Law Firm at no additional cost. Driver's License Reinstatement Assistance You and your spouse can receive 2.5 hours of legal assistance if you're having difficulty obtaining or reinstating your driver's license due to job-related or medical reasons. Personal Injury and Property Damage You and your spouse can also receive 2.5 hours of legal assistance for personal injury and property damage collection. Will Preparation Do you have a Will? As a member you can have your Will prepared at no additional charge by your Provider Law Firm. PLUS, get yearly reviews and updates at no added charge! Additionally, your spouse may have a will prepared for $50. IRS Audit Legal Services You'll receive up to 50 hours of professional services from your Provider Law Firm at no additional cost if you or your spouse receives written notice of an IRS audit of a personal tax return, or if you're requested in writing to appear at the IRS offices concerning your tax return. Moving Violation Representation Your Provider Law Firm will assist you, your spouse, and all covered family members with moving traffic violations in your personal vehicle. Uncontested Divorce Assistance Uncontested divorce assistance is available for you and your spouse after your first year of membership. Preferred Member Discount Any other legal services not otherwise covered by the membership are available at a 25% discount from the Provider Law Firm's standard hourly rate. A $50 handling fee is required for each separate legal matter. 1 Criminal charges are covered under the Preferred Member Discount. Benefits are available on the eleventh (11th) day after the date of enrollment. All benefits are subject to the following conditions and exclusions: Any fines, court costs, penalties, expert witness fees, bonds, bail bonds, and any out-of-pocket expenses are at the member's expense. Preexisting conditions, charges of DUI/DWI related matters, drug-related matters, hit-and-run related charges, leaving the scene of an accident, and unmeritorious cases are excluded. Also excluded are acts or omissions performed in the commission of any crime under any municipal, state, or federal statute, except as otherwise specifically provided for and covered within the contract. Moving and Point Violation Representation: Representation provided in the court of original jurisdiction. Representation provided for covered family members if they are properly licensed and are operating a licensed motor vehicle with no more than two axles with the express consent and permission of the owner of the vehicle. In order to receive legal services under this benefit, you must call your Provider Law Firm by phone within three (3) business days of receiving the citation. IRS Audit Legal Services: Does not cover business tax returns, garnishment, attachment or any other appeal, class actions, interventions or amicus curiae filings, charges of tax fraud or income tax evasions, Trust returns, payroll and information returns, partnerships, corporation returns or portions thereof that are included in the member's tax returns, preexisting conditions— where member has been notified by the IRS prior to enrollment, and services rendered by an enrolled agent. Personal Injury and Property Damage Assistance: Driver's license and personal injury assistance are limited to two and one-half (2- 1/2) hours of lawyer time per claim and do not include the filing of a lawsuit and exclude personal injury and property claims that exceed $2,000. Applies to claims arising as a result of driving, riding in, or being struck by any motor vehicle. Will Preparation: Trust preparation is available at the preferred member discount. Will preparation for your spouse is available for $50. Yearly reviews and updates for spouse just $50. Uncontested Divorce Assistance: Defined as a divorce where neither the member or spouse is represented by separate counsel and all issues are agreed upon by the parties, in writing, without the negotiation by the Provider Law Firm, net material assets of the marriage are under $100,000 and no division or retirement benefits is applicable. Preparation and filing of documents affecting the property ownership or encumbrances on property are not included. These services are provided at the 25% discount hourly rate. If, in their sole discretion, the Provider Law Firm determines an uncontested divorce becomes contested, subsequent benefits will then be available at the 25% discount hourly rate. All court costs and fees are the sole responsibility of the member and spouse. Preferred Member Discount: A $50 handling fee is required for each separate legal matter. Depending on your legal needs, a retainer may be required by your Provider Law Firm prior to services being rendered under this discount. Your Provider Law Firm is responsible for determining the amount of the retainer and any other anticipated costs. Other costs you may incur include fines, court costs, penalties, expert witness fees, bonds, bail bonds, and any out-of-pocket expenses. These costs are your responsibility and are not included as part of membership benefits. Your Provider Law Firm cannot provide any legal services until payment of the retainer and other costs have been made. If you need representation in court, you must notify your Provider Law Firm at least five business days in advance so they may prepare for your case.
